墨西哥签证申请表填写范本

　   办理墨西哥签证过程中关于填写申请表的要求如下：　

      -除签名外，全表必须使用英文或西班牙文填写，签名请签于“申请人姓名及签名”上面的横线上签。

　　-除右上角的编号及官方使用栏外，全表必须填写完整，包括与申请表一同提交的墨西哥签证材料、入境时间、入境城市等，否则将可能造成签证申请不被受理。

　　-白底照片请贴于左上角出现“照片”字样处。

　　-申请表各项详情填写指引请见以下：

　　I. 个人信息 PERSONAL INFORMATION:

　　该项根据护照信息填写。 Fulfill the blanks according to your passport information.

　　II. 补充信息 COMPLEMENTARY DATA.

　　请根据实际情况填写相关内容，若无请勿留空并注明没有(N/A)。Please fill in the information according to your real situation. If you don’t have any, please fill N/A.

　　17）是否合法居留指非中国公民是否在中国合法居留，若为中国公民请选“是”。 Legal residence refers to non-Chinese applicants’ legal status in China. If you have valid residential/working permit issued by Chinese authorities, please select “si”.

    III. 行程信息及目的 Travel information and purpose 20）入境日期 指的是期望入境日期，不要求确切信息。Entry date is the estimated date when you plan to enter Mexico. 21）入境城市指的是进入墨西哥后必须通过海关入境检查的第一个城市，入境城市不一定是目的地城市。Entry city means the first city you arrive in Mexico. This may not be the city you will visit, but the one in which you’ll pass the Mexican migration and customs authorities. 26）访墨目的指该次旅行的目的，请具体描述，勿笼统地填写商务访问或技术支持。Travel purpose should be described in detail,  general description such as “business visit” or “technical support” is not acceptable.

　　IV. 未成年人 underage applicants

　　Underage applicants should be accompanied by both parents if they are in China, by one of the parents if the other is absent or, if both missing, by an assigned tutor authorized by the parents. The parent who cannot be present must issue an authorization letter that clearly indicates that he/she allows the other to take their child to Mexico. This authorization letter applies to all visas application, except the ones that have a permit approved by the Mexican migration authority.

　　V. 与申请表一同递交的文件 complementary documents handed in with the application form

　　根据各人的实际情况，填写除了护照原件及申请表以外的其他补充资料。please list your complementary documents, apart from passport and application form, that you submit as documentary evidence to support you visa application, according to your unique situation.

　　VI. 签名 Signature

　　请于“申请人姓名及签名”横线之上签名，若儿童已能够书写自己的名字，监护人请勿擅自代其签名。未成年人之父母请在旁边对应的“母亲签名”“父亲签名”处签名，请勿代缺席的一方签名。Please sign ABOVE the line “Nombre y firma del solicitante”. If a child is NOT able to write his name by his own, please DON’T sign for him. Parents: please sign in the corresponding column; DON’T sign for the absent one.
